Copy of Waiver/Release of Liability

I am aware that yoga and other classes and services offered by Yoga Heights, like any physical activity, could result in injury. By my participation in classes or services offered by Yoga Heights Inc. instructors or anyone connected with the premises in any way, I agree to take full responsibility for not exceeding my physical limits and for any injury that might occur as a result of participation in class. I have fully informed the instructor of any current or previous medical condition or injury that may affect my participation in class. I hereby waive any claim I might have at any time against Yoga Heights Inc. instructors, or anyone connected with the premises in any way, for any injury or damages that may occur.

Sexual Abuse Policy: Yoga Heights prohibits and does not tolerate sexual abuse in the workplace, classroom or in any related activity. Yoga Heights requires employees, volunteers, and instructors to report sexual abuse to management. All claims will be fully investigated, and disciplinary penalties will be enacted against those who commit such acts. If clients are found to be in violation of the zero-tolerance policy, they will be prohibited from returning to Yoga Heights and forfeit any remaining balance on their account. Any Yoga Heights staff or volunteer found to be guilty of sexual abuse will be relieved of contractual obligations immediately and forbidden to return to Yoga Heights. Sexual abuse is inappropriate sexual contact of any nature.

I am 18 years or older, or will provide a copy of Yoga Heights' liability waiver signed by my legal parent or guardian prior to participation in classes or events.
